FORM	DESCRIPTION
Cattery Evaluation Form	The Cattery Evaluation form is filled out by the veterinarian who has inspected a cattery and confirmed that The Voluntary Code of Ethics has been signed.
Membership/ Cattery Application "5-years-special"	pay for 5 years and save money!
Cattery Registration	The cattery registration is a form used to register a one word cattery name which allows the breeder to use that name in their cat/litters in TICA.
ONLINE Cattery Registration	Same as above but online for your convenience. Pay by credit card or PayPal
Voluntary Code of Ethics	The Voluntary Code of Ethics is for breeders that voluntarily want to sign and agree to TICA's codes by; being a member in good standing, agreeing to breed discriminately, and also find good homes for the kittens.
Voluntary Responsible Breeder Program Packet	This packet is designed to assist you in becoming a designated TICA Voluntary Responsible Breeder.

Show License Application	In order to hold an official TICA show, this application must be submitted to the Executive Office by a chartered club 60 days prior to the show.

v Advancement to Approved Allbreed Judge	Application to advance to Approved Allbreed Judge.
Advancement to Approved Specialty Judge
Advancement to Instructor	Application to advance to Instructor status by an Approved Allbreed Judge
v Advancement to Probationary Specialty Judge	Application to advance from Trainee in the Judging Program to Probationary Specialty Judge.
v Advancement to Provisional Allbreed Judge	Application to advance from Approved Specialty to Provisional Allbreed Judge.
v Advancement to Provisional Allbreed Judge (Other Association)	Application to apply from Approved Specialty to Provisional Allbreed Judge by a judge that applied to TICA coming from another cat association.
v Agreement to Judge a TICA	Show Contract between the Cat Club hosting a cat show and the Judge Judging the cat show, sent out by the club to the judge.
Breed Comparison Report	A report written by a Trainee in the Judging Program comparing more than one cat of a breed or breed group as designated by a training instructor.
Breed Critique by Trainee	A report written on a specific cat by a Trainee in the Judging Program.
Breed Summary Sheet
Endorsement of Approved Allbreed Sponsor	A form filled out by the Approved Allbreed Judge who agrees to sponsor a trainee.
International Guest Judge
Judging Program Application	Application for admission into the TICA Judging Program
v Judging Program Application (other association)	Application to the TICA Judging Program by a judge who is or has been licensed by another domestic or international cat association.
License Request (other association)
License Request Reinstated
v Listing Form	A form provided to Trainees for tracking Breed Comparisons and Critiques
Permission to Train	A form used by Trainees for permission from the club and the training judge to train at a cat show.
Reinstatement as a TICA Judge	Application for reinstatement as a judge by a former TICA Judge.
Solo Evaluation	An evaluation form filled out by the training judge after the trainees solo training session.
Trainee Evaluation	An evaluation form filled out by the training judge after the trainees ring training session.
Trainee Monthly Report	A form sent in each month by the Trainee to the JA/designee recapping what was done the previous month.
Trainee Quarterly Report	A form sent each quarter by the Trainee to their AB Sponsor and their Regional Director recapping what was done the previous quarter.
Trainee Tracking	A form for keeping track of shows and training sessions while a Trainee is in the training program.
